$123,305 Fantasy 5 jackpot-winning ticket sold in Miami — are you the winner?

If you played Fantasy 5 last night in the Florida Lottery, then you may want to check your tickets. You could be holding onto the $123,305 jackpot-winning ticket!

The winning ticket

The jackpot-winning ticket was purchased at the Winn-Dixie, located at 1155 NW 11th St. in Miami, which happens to serve the Jackson Memorial Hospital area and Miami's Little Havana and Overtown neighborhoods.

The winning numbers for the Fantasy 5 May 2 evening draw were 10, 15, 16, 28, and 35. The winning ticket matched all five numbers to win the jackpot, which was worth $123,305. So, if you happened to play Fantasy 5 last night, your financial situation could be much better off today!

How to claim your prize

If you do happen to be holding onto this winning ticket (or any winning Lottery ticket), then you have six months to redeem your prize.

Prizes with an annual payment option have to be claimed at the Florida Lottery Headquarters in Tallahassee. You may make an appointment, but they are not required. You can visit the Florida Lottery website to schedule an appointment.

Prizes of $599 or less can be claimed at any local retailer, by drop box, by mail, or in person at any Florida Lottery claim center. Prizes of $600 to $999,999 can be claimed by drop box, by mail, or in person at any Florida Lottery claim center. Prizes of $1 million or more can be claimed by drop box, by mail, or in person at any Florida Lottery claim center.

If you are claiming a prize by drop box, you need to deposit an envelope that includes the winning ticket along with a completed claim form and a copy of your ID.

If you are claiming a prize by mail, you need to send the signed winning ticket, a completed claim form, and a copy of your ID to any claim center or directly to the headquarters located at Florida Lottery, Claims Processing, 250 Marriott Drive, Tallahassee, FL 32399-9939.

If you are claiming a prize over $50,000, then you must also include the completed Method of Prize Payment form. All of these forms and more detailed instructions on making a claim can be found on the Lottery USA website by clicking here.

Remember to keep a copy of all your claim documents for your own records.

Tax withholdings on those winnings

Keep in mind that you will be subject to tax withholdings on your winnings. In Florida, the state actually doesn’t withhold any taxes from Lottery winnings. However, you will still be subject to federal withholdings.

If you are a U.S. citizen, then you are subject to 24% federal withholding for prizes of $5,000 or more. If you aren’t a U.S. citizen, then you are subject to 30% federal withholding for prizes of $600 or more.
