To claim a prize by mail: complete and sign the back of the ticket, fill out the Prize Claim Form, and include a copy of a valid photo ID and proof of the SSN.
All documents need to be mailed via certified mail to the Wisconsin Lottery Prizes, P.O. Box 777, Madison, WI 53774. Keep a copy of all documents for your records.
Winning tickets can be claimed in person at the Madison Lottery Office by appointment only, Monday through Friday during opening times 10:00 a.m. to 2:30 p.m. To schedule an appointment call: 608-261-4916.
